Apply for Membership to the ONSIS Directory

If you provide professional, compassionate end-of-life care in Oregon, you may apply to join the ONSIS Directory. The annual fee to appear in the online Directory is $25.

Steps to Apply

Wait for Email Approval Notice

Check your junk/spam folder if you have not heard from us within 7 days.

Set Password and Pay Membership Fee

Follow the instructions in your approval email.

When all steps are complete, your listing will appear in the Member Directory. You will be able to log into your account and edit your profile.

Apply to the Directory

This is how your name will be output in the Directory (Can be first name/last name, org name, or first name/last name and org name)
Can be the same as the Contact Email
Individuals using the Directory will contact you via this email. Can be the same as your billing Email.

File should present as a square (with a 1:1 aspect ratio). Image size should be 600px x 600px or larger.
